The money you set aside in a health savings account (HSA), can be very useful when it comes to defraying all types of out-of-pocket medical costs. But those expenses aren't limited to run-of-the mill costs like copays and prescriptions. HSA funds may also be extremely helpful for qualifying individuals on even more significant expenses when eligible—such as a hearing aid or a new mattress to address sleeping issues.

In many cases, HSA funds can help pay for a variety of durable medical equipment, therapy devices and specialized supplies. Though eligibility will of course depend on the specific medical use for the item in question and your HSA plan rules, the key takeaway is that your HSA may be far more useful than you realize.

"Many of the latest tech and standalone medical services, such as health trackers and blood testing, can be purchased using HSA dollars," says R.J. Weiss, founder of the personal finance and advice platform The Ways to Wealth. Those items could feasibly include, for qualifying individuals, trending products like OURA rings, Normatec boots, and red light therapy, just to name a few.

This list of the top 50 most expensive HSA-eligible products, ranked by typical retail price, is designed to give you a better understanding of the range of high-cost items you may be able to buy with HSA cash, along with how to ensure you make that purchase correctly and obtain the proper documentation along the way.

Let's start with a brief overview about this guide. The HSA-eligible products included on our list have been ranked by typical price. Additionally, only products (not services) are included.

The items we've listed represent a range of categories including medical equipment, fitness equipment, health technology, sleep supplies and more.

The items chosen for inclusion can be considered a qualified medical expense when used for medical care, but it's important to note that a Letter of Medical Necessity (LMN)from a healthcare provider is often required for full HSA coverage and these purchases are only eligible when prescribed to treat a diagnosed medical condition.

The money accumulated in your HSA fund is meant to be used for qualified medical expenses. But understanding what's qualified and what's not can sometimes be confusing. IRS publication 502 spells this out in more detail explaining that qualified means "expenses that would generally qualify for the medical and dental expenses deduction." But even that statement doesn't really nail it down with complete clarity.

The good news is that IRS further explains that eligible expenses would encompass: "The costs of diagnosis, cure, mitigation, treatment, or prevention of disease, and for the purpose of affecting any part or function of the body." And here's the key passage of explanation in Publication 502, at least as it relates to high-ticket purchases: "These expenses include …the costs of equipment, supplies, and diagnostic devices."

In addition to these IRS rules, there are some important nuances to understand before making a big ticket purchase. For instance, a product that can be used for both medical needs and general purpose may require documentation from a medical professional in order to be covered by HSA spending.

"Some higher ticket items may require a letter of medical necessity, especially if the product could be considered dual-purpose—for medical versus general health. But when properly documented, these purchases can be a very efficient use of HSA dollars," says Ethan Pickner, a health insurance broker with AZ Health Insurance.

Another important point that can lead to a purchase that may not be covered: While many retailers often helpfully label products that are "HSA eligible", those labels are also not a guarantee that the purchase you're about to make is covered in all cases.

"HSA eligibility is driven by how and why a product is used, not just how it’s labeled,"says Jason Winkelmann, ND, of True Health Centers. " The same item may be eligible in one context and not in another."

Additionally, many costly purchases require a prescription or LMN from a health professional explaining why a particular product, treatment, or service is necessary to cure, treat, mitigate, or prevent a health or medical condition, in order to be eligible for full HSA coverage or reimbursement.

The bottom line before making a big ticket purchase is that it's always best to take the time to check with your HSA plan administrator first, before spending money, in order to confirm that a purchase is definitely eligible. Taking this step can help you avoid making accidental, ineligible purchases.

Now that you have a better idea of the range of big ticket items that may be eligible for HSA coverage, let's talk about how to identify the best HSA product for your needs.

The first step, when considering using HSA funds for a costly purchase, is to pinpoint the health problem you want to solve. In other words, what symptom or condition are you seeking to deal with or treat? Answering that question makes it far simpler to identify the most appropriate product or device.

"Patients should ask 'What condition am I trying to improve, and what tool best supports that goal?'" says Winkelmann. "From there, important considerations include clinical effectiveness, durability, ease of use, and whether the product addresses the root issue, rather than just symptoms."

Also bear in mind that similar to shopping for a television, refrigerator or any other significant purchase, that a product's features, quality, accuracy, materials and more will impact the price you'll ultimately pay. For this reason, it's important to do your research in advance to make the most informed purchase possible. This includes determining which features are important to you, along with how materials affect a product's durability.

And finally, ensuring that you fully understand return policies is critical before spending a great deal of money on a purchase. Be sure to read the fine print on this front or speak with the product manufacturer if you have questions about the return policy. It's equally important to find out whether the product comes with a warranty, and if so, what it covers, along with how long the warrant remains in place.

Many HSA plan administrators issue HSA cards that can be used to make purchases for HSA-eligible items much like you would with a bank-issued debit card. You can swipe the card at the point of purchase in person at a store (many big box retailers accept HSA cards) or use it to complete a purchase online just as you would with a bank issued debit card.

When making a purchase with your HSA debit card, be sure to keep documentation surrounding your purchase. If you're making the purchase online this would include the order confirmation, itemized receipt and even a screenshot of the product description.

HSA debit cards however, are only designed to work at merchants that provide eligible items. And in some cases, incorrect or inaccurate merchant coding can cause the purchase to be declined.

One more important note, if you're shopping online and your cart includes a mix of both HSA-eligible and ineligible items, you may be able to use what's known as the 'split tender' strategy to complete the purchase. Some merchants support this approach which involves using your HSA card for the eligible portion of the purchase and a personal card for the remainder of the purchase. However, not all merchants support this approach.

Maintaining documentation is an important step when using HSA funds for purchases, especially big ticket items. This includes hanging on to the receipt from the merchant where you make a purchase. Alternatively, if you're shopping online, take a screenshot of the purchase confirmation or make a PDF of the receipt you receive.

The purchase receipt should ideally include the product name, purchase date, and total cost of the item you bought. This documentation is important should any questions arise later about whether the purchase was eligible for HSA coverage. Receipts also come in handy should your HSA administrator ask for supporting documentation related to the purchase. They're also important for tax season.

Another important compliance note, don't seek HSA coverage or reimbursement for a purchase that was already covered elsewhere, such as with flexible spending arrangement (FSA) funds. Seeking coverage from both accounts for the same expense is known as double-dipping and its forbidden by the IRS.

Buying a general purpose version of a product, rather than the medical purpose version, is another common pitfall. This error can result in the purchase not being covered by your HSA.

Failing to obtain an LMN prior to making a purchase can also trigger issues down the road. "Making a purchase without first having a Letter of Medical Necessity can easily pose a problem should such a purchase be subject to later scrutiny," says Scott Dowling, an HSA expert, insurance coach and host of the podcast Doxcost, The Ultimate Guide to Health Insurance.

"Not having an LMN,” adds Dowling, “along with poor record keeping, will most certainly pose a problem in the future."

Dowling's point is an important one. When making HSA purchases, be sure not to throw away or lose receipts. Responsibly maintaining documentation is important should there be any questions about your purchase from the HSA plan administrator or the IRS at a later date.

Another all-too-common pitfall to avoid is using HSA cash for someone who isn’t a qualified dependent. Per IRS regulations, HSA money can only be used to pay for your spouse or qualified dependent's expenses. Moreover, a dependent is only eligible in cases where you pay at least 50 percent of their financial needs.
