Every LMN is an individualized clinical decision by an independent licensed practitioner. There is no automation and no guaranteed approval. LMNs are administrator ready. Each LMN includes the medical condition with ICD-10 codes, clinical rationale with evidence-based research citations, a specific treatment plan (frequency, duration, role within the care plan), and the licensed practitioner's signature confirming medical necessity.
